RecycleHealth wants to learn more about your interest in using a wearable activity tracker.
To request a wearable activity tracker, please take our survey. We ask that if you are willing to be a part of our research, please complete the full survey, which should take about 10 minutes. At the end of the survey, if you opt-in, we will send a follow-up survey in 6 weeks, so we can see how your wearable has helped you! You are free to stop taking the survey at any time you want, without affecting whether you receive a tracker. You are not required to be a part of our research to get a tracker. If you are not interested in taking the full survey, please complete the first two pages only, then click to the end of the survey and submit your request.
